All-inclusive elder care combines medical, personal, and social support into one coordinated program designed to help older adults remain safely at home. Unlike traditional services that are often fragmented, this model provides everything from primary care and therapies to meals, social activities, and transportation — all managed by one team.

Empath Health offers four all-inclusive elder care programs across Florida. Three are PACE (Program of All-Inclusive Care for the Elderly) centers, and one specializes in veterans care. Each program is tailored to the unique needs of its community while remaining part of our larger Full Life Care network.

Eligibility depends on the program. Our PACE centers serve adults age 55+ who meet clinical criteria for nursing home-level care and wish to remain in their homes. The Auer-Szabo Empath Adult Day Center serves adults age 18 and older, with services tailored to individuals who need daily support in a structured, social environment. Our team can help determine the right fit based on each person’s needs and circumstances.

Participants are supported by an interdisciplinary team that may include physicians, nurses, social workers, therapists, personal care aides, activity coordinators, and transportation staff. The team works closely with each participant and their family to deliver personalized, ongoing care.

These programs reflect Full Life Care by honoring the whole person — their health, independence, relationships, and purpose. Whether it’s a veteran receiving specialized care or a participant enjoying social time at a day center, every moment is supported by a system that sees people as more than their diagnoses.

Anyone — a family member, healthcare provider, or potential participant — can reach out to us directly to learn more or begin the referral process. Volunteers and donors are also essential to these programs, helping enrich activities and expand access to care.

Covered services may include doctor visits, medications, therapies, transportation, meals, personal care, adult day services, and hospital or nursing care when needed. The care team coordinates all of this to remove financial and logistical burdens for participants and families.

Yes, when someone enrolls in Empath LIFE, their care is managed by our dedicated team. This means switching from outside providers to our program's healthcare professionals — but it also means better coordination, faster access to services, and a team that truly knows the participant. If a specialist is needed, we'll arrange that too, as part of the seamless care model.

Yes. Safe, reliable transportation is a core part of our programs. We provide rides to and from the day center, medical appointments, and other essential services. Our drivers are trained to support participants with mobility needs, ensuring they feel comfortable and cared for from door to door.
