What does dementia care look like at Empath Health?

Empath Health provides dementia care that supports individuals and care partners from the earliest signs through the most advanced stages. Our services include care planning, caregiver education, behavioral and emotional support, respite options, and end-of-life guidance — all designed to preserve quality of life and connection at every step.

What is Empath GUIDE?

Empath GUIDE is our dementia support program that helps individuals and their care partners understand and navigate a diagnosis. It offers personalized guidance, education, and connections to resources that reduce stress, build confidence, and extend independence. GUIDE is built around the belief that no one should go through this journey alone.

What is the Nancy Renyhart Dementia Education Program?

The Nancy Renyhart Dementia Education Program (NRDEP) is an initiative of Empath Health dedicated to raising awareness, increasing understanding, and improving community-wide support for those affected by dementia. Through trainings, public presentations, and caregiver-focused events, NRDEP helps equip professionals and families with the knowledge and tools they need.

Who can access these dementia services?

Many of our dementia care services are available to anyone in the community, regardless of age or stage of diagnosis. We support patients and families receiving care through Empath programs — including hospice, elder care, and home health — and also serve individuals not currently enrolled in our clinical services. Our team can help guide you to the right level of support.

How does this reflect Empath’s Full Life Care model?

Dementia affects more than memory — it impacts identity, relationships, and everyday life. Our approach is rooted in Full Life Care, which means we address the whole person and their circle of care, not just the disease. From meaningful activities to emotional support and forward planning, our goal is to help people live fully through every chapter.

How can I get involved or support Empath’s dementia programs?

There are many ways to support this work — from volunteering with education programs and support groups to donating to expand access to services and build capacity. Community members, health professionals, and caregivers alike are invited to help us create more dementia-capable communities across Florida.

Today, dementia affects many people. It can take several years to progress and cause numerous health and function issues, including the inability to communicate or make decisions. Planning ahead for the medical treatments you would prefer is important. Our Dementia Living Will Extension is a specifically designed communication tool documenting the kinds of medical care and quality of life you want if you develop or have moderate or severe dementia. It becomes effective when you are no longer able to make medical decisions for yourself as comes with advanced dementia.
