CLEARWATER — Empath Health announced today the appointment of Janet Roman, DNP, RN, APRN, ACNP-BC, CHFN, ACHPN as the company’s new Director of Cardiac Programs. Roman will oversee the development and implementation of the new Home Health Care program focused on patients with advanced cardiac disease as part of its ongoing efforts to expand the Empath Health integrated network of services.
The Heart Failure Palliative Home Care program will give cardiologists the confidence to refer their complex cardiac and heart failure patients to Empath Health, knowing they can optimally and seamlessly support those patients at home or in facilities with specially-trained home health, palliative care, and hospice support teams.

The new Director of Cardiac Programs has a doctorate in nursing practice with board certification as an acute-care advanced practice registered nurse (APRN) and nursing certifications in advanced heart failure and advanced hospice and palliative care. She has extensive experience in the development of cardiac disease management and palliative care programs across the continuum of acute and post-acute care.
Roman was instrumental at Abington-Jefferson Health System (Philadelphia) in creating a program that successfully bridged inpatient, outpatient, home care and hospice settings for patients with advanced heart failure.
Roman also worked at the Department of Veterans Affairs (New Orleans), where she helped expand palliative care services – with a special focus on veterans with advanced cardiac disease across a variety of care settings
